Job summary

Tender Touch Rehab Services LLC is hiring a Licensed Occupational Therapist for a full-time position in a skilled nursing/long-term care setting. You will evaluate and treat patients, design personalized care plans, and work with a multidisciplinary team to ensure optimal functional outcomes.

Qualifications

• OT degree from an AOTA-accredited program.
• NBCOT certification requirement as OTR where state requires.
• Current license to practice as Occupational Therapist.

Responsibilities

• Evaluate and treat patients with functional impairments and ADLs.
• Create and implement personalized care plans.
• Collaborate with interdisciplinary team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are current and obtained.

What You’ll Do:

• Evaluate and treat patients with functional, cognitive, and ADL impairments
• Create and implement personalized care plans
• Collaborate with team members to ensure quality outcomes
• Maintain documentation to meet all regulatory requirements
• Ensure all required physician orders are obtained and up to date

Qualifications:

• Degree in Occupational Therapy (BS, MS, MA, or OTD) from an AOTA-accredited program
• Certified by the NBCOT as an OTR, if required by the state
• Current license as an Occupational Therapist in the state of practice
• Strong communication and organizational skills
