Peppercare is seeking a Home Health Occupational Therapist to provide skilled therapy in patients' homes in Ventura, CA. You will deliver personalized care to improve independence and quality of life, with mentorship available for newcomers to home health.

Qualifications

• California Occupational Therapist license
• Current CPR/BLS-Healthcare Provider certification
• Current driver’s license and automobile insurance may be required

Responsibilities

• Assess rehabilitative needs of home-bound clients with comprehensive evaluation and treatment plan
• Promote independence in the home through therapeutic intervention and adjust treatment plan as needed
• Educate patients and families on home exercise programs, safety and falls prevention
• Supervise assistants as needed
• Communicate effectively with all team members
• Maintain patient records in compliance with federal and state regulations and company policy

Job Summary
