Job summary

Peppercare in Ventura, CA is seeking a compassionate Home Health Speech Therapist to join our growing team. In this role you will deliver personalized therapy in patients’ homes, helping them recover and regain independence with a patient-centered approach.

You will work with families and caregivers, develop treatment plans, and coordinate with a multidisciplinary team. A Master’s in Speech-Language Pathology and California license are required, with mentorship offered for new home health

Qualifications

• Master’s degree in Speech-Language Pathology required.
• California Speech-Language Pathology license required.
• CCC-SLP certification is required.
• Valid driver's license and automobile insurance.
• Current CPR/BLS for healthcare providers.
• Current TB test clearance.

Responsibilities

• Assess speech, cognitive and swallowing needs and create a treatment plan.
• Provide therapy to improve speech, communication and swallowing abilities.
• Educate patients and caregivers on home exercise programs and safety.
• Communicate effectively with all care team members.
• Maintain patient records per federal/state regulations and policy.

Job Responsibilities

• Assess the speech, cognitive and swallowing needs of our home-bound clients with comprehensive evaluation and treatment plan
• Promote speech, communicative and swallowing abilities through therapeutic intervention and modify treatment plan as needed
• Educate patients, families, caregivers regarding home exercise program, safety
• Communicate effectively with all team members
• Maintain patient records in accordance with federal and state regulations and company policy to ensure quality standard of care.
