Peppercare seeks a dedicated Home Health Physical Therapist to provide in-home therapy, helping patients recover and regain independence. You will work with clients in their homes, delivering personalized care and promoting best practices in home health therapy.

Formación

• California Physical Therapist license.
• Current driver's license.
• Automobile insurance.
• Current CPR/BLS-Healthcare Provider.

Responsabilidades

• Assess the rehabilitative needs of home-bound clients with comprehensive evaluation and treatment plan.
• Promote independence in the home setting through therapeutic intervention and modify treatment plan as needed.
• Educate patients, families, caregivers regarding home exercise program, safety, falls prevention.
• Supervise Assistants as needed.
• Communicate effectively with all team members.
• Maintain patient records in accordance with federal and state regulations and company policy to ensure quality standard of care.
