Job summary

Peppercare is seeking a compassionate Home Health Occupational Therapist to deliver patient-centered care in the homes of clients in Thousand Oaks and surrounding areas. You will perform evaluations, design and implement treatment plans, and educate patients and families to promote independence and safety at home.

Qualifications

• Master's degree or higher in Occupational Therapy.
• California OT license is required.
• Experience in home health is preferred.

Responsibilities

• Assess rehabilitative needs of home-bound clients and develop treatment plans.
• Promote independence in the home through therapeutic interventions.
• Educate patients, families, and caregivers on home exercise programs, safety, and falls prevention.
• Supervise assistants as needed.
• Communicate effectively with the care team.
• Maintain patient records per federal and state regulations and company policy.

Job Responsibilities

• Assess the rehabilitative needs of our home-bound clients with comprehensive evaluation and treatment plan
• Promote independence in the home setting through therapeutic intervention and modify treatment plan as needed
• Educate patients, families, caregivers regarding home exercise program, safety, falls prevention
• Supervise assistants as needed
• Communicate effectively with all team members
• Maintain patient records in accordance with federal and state regulations and company policy to ensure quality standard of care.

Education

• Undergraduate or Masters Degree

Required Certification

• Current Physical/TB
