Job summary

Sweetwater Care and Rehab is seeking an Occupational Therapist to join our in-house therapy team. The role focuses on providing rehabilitative therapy to patients referred for cognitive and/or ADL physical functioning disorders, with emphasis on recovery, function improvement, and return to prior levels of functioning.

The position reports to the Director of Rehabilitation Services and requires active State Licensure and adherence to NBCOT/AOTA professional standards and state practice acts.

Qualifications

• Active State Licensure.
• Adherence to NBCOT / AOTA and State Required practice acts, code of conduct, and ethics.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulatory requirements.
• Administer procedures as part of the rehabilitation plan (ADL retraining, therapeutic activities, exercises, assistive devices).
• Assess progress and adjust treatments in the patient record per requirements.
• Evaluate treatment outcomes and refer as appropriate based on results.
• Provide counseling to patients and families about physical disorders.
• Communicate progress, barriers, and plans with supervisor and health team.
• Provide comprehensive discharge summaries per regulatory requirements.
• Document all required information in electronic systems and maintain records.
• Record treatment minutes accurately and participate in care conferences and trainings.
• Supervise and support occupational therapy assistants.
• Comply with professional conduct acts during practice.
