Job summary

Vertis Therapy is seeking a compassionate Occupational Therapist for our Skilled Nursing Facility in Middletown, IN. You will evaluate residents to improve independence and quality of life, and develop individualized treatment plans.

Responsibilities include documenting progress, supervising COTAs as needed, and participating in team development meetings and wellness events to support resident well-being.

Qualifications

• Degree in Occupational Therapy from an accredited institution.
• Current OT license or license eligibility in Indiana.
• Strong clinical assessment and ability to develop effective treatment plans.

Responsibilities

• Evaluate residents' functional abilities and develop plans of care to improve independence.
• Provide skilled occupational therapy services and monitor progress.
• Supervise Certified Occupational Therapy Assistants as appropriate.
• Lead or participate in team development meetings and wellness programs.
