Job summary

Vertis Therapy is seeking a passionate Occupational Therapist for PRN hours at our Assisted Living in Bloomington, IN. The OT evaluates and treats patients under physician orders to improve independence, safety, and quality of life.

Responsibilities include comprehensive assessments, individualized treatment planning, documentation, and collaboration with nursing staff and families. Requires an OT license and NBCOT registration preferred.

Qualifications

• Master of Science degree in Occupational Therapy or Bachelor of Science degree in Occupational Therapy with five years of direct practice experience.
• Current license/certification in the state of practice.
• Prefer current registration with the American Occupational Therapy Certification Board (NBCOT).
• Demonstrated competency in applying OT principles, methods, materials and equipment to the patient population served by this facility.
• Excellent oral and written communication and interpersonal skills.
• Adequate computer skills to complete required job duties and documentation.
• Ability to assess patient needs and develop and implement a comprehensive plan of care.
• Knowledge of accreditation standards and compliance requirements.
• Promote Vertis Therapy programs and services in the community.

Responsibilities

• Review patient information and assess impact on the treatment process.
• Conduct thorough functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Comply with evaluation, treatment and documentation guidelines.
• Record patient progress at least every seven days.
• Revise treatment plans as needed during treatment.
• Document course of treatment clearly, including discharge summaries.
• Instruct and supervise assistants during treatment procedures.
• Orient patients to services and inform about potential risks.
• Work with nursing staff and families to maximize functional abilities.
• Educate patients and families for continued care after discharge.
• Assist in referral processes after discharge and maintain knowledge of community resources.
• Attend rehabilitation team conferences and other department meetings.
• Engage in continuing education to maintain clinical accuracy.
• Contribute input to Rehab Director on assistants' clinical performance.
