Job summary

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop a plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled treatments per regulatory and clinical requirements.
• Administer procedures as part of the rehab plan (ADL retraining, therapeutic activities, exercises, modalities).
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed based on results.
• Offer counseling to patients and families; coordinate with the care team.
• Communicate progress and plans with supervisors and health teammates.
• Provide comprehensive discharge summaries per regulatory requirements.
