You will assess cognitive and ADL disorders, develop treatment plans, and collaborate with a multidisciplinary team to support geriatric patients on their recovery journey.

Qualifications

• Licensed Occupational Therapist with active license.
• Open to applicants of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL impairments and develop care plans.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled treatment to patients per regulatory requirements.
• Administer ADL retraining, therapeutic activities, exercises and modalities.
• Assess progress and adjust treatments in clinical records.
• Evaluate treatment outcomes and refer as needed.
• Counsel patients and families about physical disorders.
• Communicate progress with supervisor and health team.
• Provide discharge summaries per regulatory requirements.
