Job summary

Rivetus Rehabilitation is seeking a licensed Occupational Therapist to join our community-based care team in Troy, MI. You will support two programs: Rivetus Bridge Mobile Outpatient and Take Your Therapist Home Home Health, with a consistent caseload and clinician autonomy.

You will assess and treat patients, develop individualized plans, supervise OTAs, document thoroughly, and collaborate with a dedicated clinical team to optimize independence and quality of life for our geriatric population.

Qualifications

• Accredited Occupational Therapy program completed.
• Current Michigan licensure required.
• New graduates welcome to apply.
• Ability to work in a community-based setting with autonomy.
• Strong documentation and CQI participation.

Responsibilities

• Perform OT screenings and evaluations for ADLs, work, and leisure.
• Develop and implement individualized treatment plans.
• Provide OT services per AOTA standards.
• Deliver care through Mobile Outpatient and Home Health in Oakland County.
• Direct patient care and maintain productivity standards.
• Coordinate visits and manage schedule with clinician autonomy.
• Supervise OTAs and aides as assigned.
• Complete timely documentation and support care continuity.
• Participate in meetings, CQI activities, and program development.
