Job summary

Rivetus Rehabilitation in Michigan is seeking a licensed Occupational Therapist for a community-based role serving geriatric Post-Acute patients. You will provide OT services through two Rivetus programs: Rivetus Bridge Mobile Outpatient and Take Your Therapist Home Home Health, with a consistent caseload and centralized Oakland County coverage.

• Current state licensure in Michigan.

Responsibilities

• Provide OT services across two Rivetus programs: Mobile Outpatient and Home Health.
• Assess, plan, and implement treatment to maximize independence in daily activities.
• Document care and coordinate with the clinical team.
• Supervise OTAs and aides as assigned.
• Maintain timely documentation for quality and reimbursement.
• Participate in CQI, continuing education, and team meetings.
• Schedule management with autonomy within centralized territory.
