Job summary

Rivetus Rehab in Troy, MI is seeking an Occupational Therapist for Mobile Outpatient/Home Health to join our geriatric rehab team. You will manage a consistent caseload, deliver skilled OT services under AOTA standards, and support patient independence across daily living activities.

You will supervise OTAs and aides, document thoroughly, coordinate care, and participate in program development across Rivetus Bridge and Take Your Therapist Home initiatives, with flexible full-time schedules.

Qualifications

• Successful completion of an accredited OT program.
• Current Michigan state licensure required.
• New graduates encouraged to apply.

Responsibilities

• Conduct OT screenings and evaluations to assess daily living independence.
• Develop and implement individualized treatment plans.
• Provide skilled OT services per AOTA standards.
• Treat patients through Mobile Outpatient and Home Health programs.
• Coordinate patient visits and manage schedule with clinician autonomy.
• Supervise OTAs and aides as assigned and ensure proper documentation.
• Participate in CQI activities, continuing education, and program development.
