Job summary

Rivetus Rehabilitation in Michigan is seeking a licensed Occupational Therapist for a community-based role serving geriatric Post-Acute patients. You will provide OT services through two Rivetus programs: Rivetus Bridge Mobile Outpatient and Take Your Therapist Home Home Health, with a consistent caseload and centralized Oakland County coverage.

Schedule: Full Time - Flexible Schedules Available!

General Summary:

In this unique community-based role, you will provide occupational therapy services through two Rivetus programs: "Rivetus Bridge" Mobile Outpatient and "Take Your Therapist Home" Home Health. This position offers a consistent caseload, centralized Oakland County coverage, patient continuity, and the autonomy to build a flexible schedule. Responsibilities include patient assessment, treatment planning and implementation, documentation, care coordination, and supervision of OTAs and aides as assigned.

Your Responsibilities:

• Conduct occupational therapy screenings and evaluations to determine patient independence with activities of daily living, including self-care, work, and leisure.
• Develop and implement individualized treatment plans using adaptations, strengthening, graded activities, and retraining to maximize independence.
• Provide skilled OT services consistent with AOTA standards and current clinical practice.
• Treat patients through both Mobile Outpatient and Home Health programs within a centralized Oakland County territory.
• Provide direct patient care in accordance with clinical and productivity standards.
• Coordinate patient visits and manage your schedule with a high degree of clinician autonomy.
• Supervise, instruct, assign, and review the work of OTAs and aides as assigned.
• Complete accurate and timely documentation supporting quality, continuity of care, and reimbursement.
• Participate in patient care conferences, CQI activities, continuing education, in-services, and program development as requested.
• Participate in company meetings and activities supporting effective departmental operations.

Required Qualifications:

• Successful completion of an accredited Occupational Therapy program.
• Current licensure as required by the State of Michigan.
• New graduates are encouraged to apply.
