Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. This position offers Full Time, Part Time, or PRN options and focuses on planning, organizing, and directing OT services in accordance with federal, state, and local standards, guidelines, and regulations, under the direction of the Administrator to ensure the highest quality resident care.

The role includes screening residents, developing treatment plans with goals,

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team members.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to treatment plans and ensure proper documentation and billing.
