Job summary

Evolve Therapy Services, LLC in Pahokee, FL seeks an Occupational Therapist to join an in-house skilled nursing facility team. You will assess residents, plan therapy, and collaborate with an interdisciplinary team to deliver high-quality care.

This position offers flexible scheduling (Full Time, Part Time, or PRN), with opportunities for growth. An active Florida OT license is required, plus ongoing continuing education to maintain licensure and ensure compliant care and documentation.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Deliver therapy as per plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments align with reimbursement practices.

Education

Degree in Occupational Therapy

Active OT license in Florida
