Job summary

Evolve Therapy Services, LLC is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. The role supports full-time, part-time, or PRN schedules to deliver quality therapy services.

You will plan, organize, develop, and direct OT services, ensuring alignment with federal and state standards, while coordinating with administrators and the care team to uphold high resident care standards.

Qualifications

• Must hold an active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.
• Degree in Occupational Therapy is required.

Responsibilities

• Conduct regular screening of residents to determine best interventions.
• Evaluate residents to inform treatment plans with short- and long-term goals.
• Interpret evaluation findings and communicate plans to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet clinical standards and reimbursement practices.
