Concept Rehab is seeking a dedicated Occupational Therapist (OT) to join our therapy team in skilled nursing and long-term care settings. Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled occupational therapy services.
• Help patients improve daily living skills, mobility, cognition, strength, and functional independence.
• Monitor progress and adjust treatment plans as needed.
• Supervise OT Assistants (OTAs).
• Collaborate with physicians, nurses, families, and interdisciplinary team members.
• Participate in care plan and discharge planning meetings.
• Educate patients, caregivers, and facility staff.
• Recommend equipment and interventions to improve patient outcomes.
• Maintain timely, accurate documentation and billing.
• Comply with state licensure, regulatory, and professional practice standards.
