Job summary

Goldsboro Rehabilitation And Healthcare Center in Goldsboro, NC is seeking an Occupational Therapist to perform evaluations and develop treatment plans for residents in long-term care. The OT will work with an interdisciplinary team, document progress, ensure orders and documentation are accurate, and provide patient-centered therapy using approved modalities.

Candidates should have an OT degree recognized by AOTA, be licensed in the state, and SNF experience is preferred.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• SNF experience preferred.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• Document patient's daily participation and progress on progress note accurately and timely.
