Job summary

Qualifications

• Graduate of an accredited university with a degree in Occupational Therapy.
• Current license/registration in the state.
• SNF experience preferred.

Responsibilities

• Provide comprehensive occupational therapy evaluations based on physician orders.
• Develop and update treatment plans with long and short-term goals, frequency, duration, and modalities.
• Document evaluations, progress, discharge summaries, and required forms in a timely, accurate manner.
• Coordinate with interdisciplinary team and participate in facility meetings and care planning.
• Supervise staff and students as needed and maintain professional relationships with patients and families.
