Job summary

Goldsboro Rehabilitation And Healthcare Center in Goldsboro, NC is seeking a full-time Occupational Therapist to join our rehabilitation team. You will conduct comprehensive OT evaluations, develop treatment plans, and document patient progress in coordination with MD orders and facility policies.

Responsibilities include therapy delivery, discharge planning, and participation in inter-disciplinary meetings while maintaining professional patient relations and timely documentation.

Qualifications

• OT degree (B.S./M.S./D.P.T.) recognized by AOTA.
• SNF experience preferred.
• Holds current OT license/registration in the state.

Responsibilities

• Provide comprehensive OT evals based on MD orders.
• Document findings on standardized evaluation forms.
• Create comprehensive treatment plans with goals, frequency and modalities.
• Ensure MD orders for evaluations, treatments, recertifications and discharges.
• Provide treatments with appropriate modalities and safety precautions.
• Document daily participation and progress in progress notes.
• Update treatment plan monthly on recertification.
• Prepare discharge summaries and in-service documentation as required.
• Complete facility documentation and MDS-related information as needed.
• Attend rehab meetings and participate in inter-disciplinary team.
