Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is hiring an Occupational Therapist (OT) for full-time, part-time, or PRN options. Join a dedicated rehab team and impact resident outcomes with comprehensive evaluations and functional treatment planning.

Responsibilities include conducting OT evaluations, developing treatment plans, documenting progress, and collaborating with the interdisciplinary team to deliver quality patient care in a skilled nursing setting.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• SNF experience preferred.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Provide a comprehensive treatment plan including goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertification and discharges.
• Provide comprehensive treatment to patients utilizing modalities and adjust treatment as needed.
• Document patient progress and update treatment plans and discharge summaries as indicated.
• Co-sign notes and supervise junior staff as needed.
• Maintain professional relationships with staff, patients and families.

Skills

Current OT license in NC

SNF experience preferred

Education
