Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC, is hiring an Occupational Therapist (OT) for full-time, part-time, or PRN roles. The position offers a $10K relocation bonus and a competitive wages and benefits package.

Responsibilities include evaluating patients, developing treatment plans, documentation, coordination with MD orders, and collaboration with the rehab team. SNF experience and an active OT license are preferred.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy that is recognized by the AOTA.
• SNF experience preferred.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertification and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• Document patient’s daily participation and progress on progress note accurately and timely.
• Document monthly updated treatment plan on recertification comprehensively and in a timely manner.
• Document patient’s discharge on a discharge summary as indicated.

Education

BS/MS/MA/MS or Doctorate in Occupational Therapy

OT license in good standing (state)
