Job summary

Hertford Rehabilitation And Healthcare Center seeks an Occupational Therapist to join a dedicated rehab team in Hertford, NC. Full time, part time, or PRN positions available with relocation bonus and a competitive wages package.

You will evaluate residents, develop and implement treatment plans, document progress, and collaborate with an interdisciplinary team to improve long-term care outcomes.

Qualifications

• Graduate with OT degree recognized by the AOTA.
• SNF experience preferred.
• Hold current OT license/registration in the state.

Responsibilities

• Provide comprehensive OT evaluations based on physician orders.
• Document findings and adhere to evaluation guidelines.
• Develop/adjust treatment plans with long- and short-term goals and modalities.
• Ensure orders are obtained for all evaluations and treatments.
• Deliver comprehensive therapy and document progress weekly and monthly.
• Maintain discharge summaries and update care plans as needed.
• Attend meetings and participate in interdisciplinary care planning.
• Supervise staff and students as required and co-sign notes.
• Maintain professional relationships and educate staff as needed.
