Job summary

Renewal Rehab in Baraboo, WI is seeking a licensed Occupational Therapist (OT) to provide comprehensive OT services. The role involves evaluations, treatment planning, and delivering patient-centered care under physician orders.

Qualifications

• Must be licensed and/or registered Therapist to apply.
• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy.
• NBCOT designation as OTR (if state mandated) and active OT license/registration.

Responsibilities

• Provide comprehensive Occupational Therapy evaluations based on MD orders.
• Document findings timely on standardized evaluation forms.
• Create comprehensive treatment plans with goals, frequency, duration, and modalities.
• Ensure MD orders for evaluations, treatments, recertifications, and discharges.
• Deliver treatment utilizing modalities and adjust as indicated while following precautions.
