Renewal Rehab in Baraboo, WI is seeking a licensed Occupational Therapist (OT) to perform comprehensive evaluations and deliver patient-centered care per MD orders. You will formulate treatment plans, document findings, and coordinate discharge planning with the medical team.

Qualifications

• Licensed and/or registered Occupational Therapist.
• NBCOT designation as OTR (if state mandates).

Responsibilities

• Provide comprehensive occupational therapy evaluation based on MD orders.
• Document findings on standardized evaluation forms timely and accurately.
• Develop comprehensive treatment plan with goals, frequency, duration, modalities.
• Ensure MD orders obtained for evaluations, treatments, recertifications, discharges.
• Deliver comprehensive treatment utilizing modalities with precautions.
