Job summary

Renewal Rehab in Baraboo, WI is seeking a licensed Occupational Therapist (OTR) to join our growing team as a PT. You will perform evaluations, develop treatment plans, and deliver therapy in accordance with MD orders and professional guidelines.

Qualifications

• Graduate of an accredited OT program with NBCOT-OTR designation.
• Current Wisconsin OT license/registration.
• Must be licensed and/or registered to practice.

Responsibilities

• Provide comprehensive occupational therapy evaluations based on physician orders.
• Document findings timely on the standardized evaluation form.
• Provide a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver comprehensive treatment to patients using modalities and adjust as indicated.

What You'll Do:

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short-term goals, frequency, duration, and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.

Qualifications:

• Graduate of an accredited university with a B.S.,MA, or M.S. or Doctorate in Occupational Therapythat the AOTA recognizes.
• Recognized by the NBCOT as a designated OTR (if state mandated)
• OT holds a current license and/or registration as an Occupational Therapist in-state as applicable.
