Responsibilities

• Screens/evaluates patients with cognitive and/or ADL functional disorders and develops a plan of care.
• Uses standardized assessments to support interventions.
• Provides rehabilitative and skilled treatment per standards.
• Administers procedures as part of the rehabilitation plan.
• Assesses progress and adjusts treatments in the clinical record.
• Evaluates outcomes and refers as needed.
• Provides counseling to patients and families.
• Communicates progress with the team.
• Delivers comprehensive discharge summaries.
