Job summary

Irondale Post Acute in Commerce City, CO is seeking an Occupational Therapist for full-time and PRN schedules to join our in-house rehab team serving geriatric patients. You will screen and evaluate patients, develop care plans, and deliver skilled rehabilitative interventions while adhering to regulatory standards.

Qualifications

• Occupational Therapist license is required.
• Open to Occupational Therapists at all experience levels. New Grads are welcome!

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver rehabilitative, skilled and medically necessary interventions.
• The role involves regular standing and walking; occasional lifting, transferring, repositioning of patients and equipment.
• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, assistive devices and wound care.
• Provide consultation to patients, families, caregivers and other service providers regarding physical disorders.
• Generate discharge summaries of services provided, ensuring regulatory compliance.
