Heights Care and Rehab is seeking a compassionate Occupational Therapist (OT) to help residents regain skills for independence. You will evaluate residents, develop personalized treatment plans, and collaborate with a multidisciplinary team to support safe transitions home.

We offer flexible scheduling (full-time, part-time, PRN) and competitive pay. A sign-on bonus is available and continuing education support is provided to promote professional growth.

Competenze

• Colorado license required as an OT.
• Graduate of an accredited Occupational Therapy program.
• Excellent communication and interpersonal skills.
• Strong clinical reasoning and problem-solving abilities.
• New graduates are encouraged to apply.

Mansioni

• Evaluate residents and develop individualized treatment plans.
• Help residents perform activities of daily living (ADLs).
• Address deficits in strength, coordination, cognition, safety awareness, and independence.
• Recommend adaptive equipment and environmental modifications.
• Collaborate with nursing, therapy, physicians, families, and interdisciplinary team members.
• Assist with discharge planning and safe transitions home.
• Educate residents and caregivers on safety and independence techniques.
• Maintain timely and accurate documentation.
• Participate in care planning and interdisciplinary meetings.
