Job summary

Cantex Continuing Care Network is seeking an Occupational Therapist for Colonnades (Pearland) and Laurel Court (Alvin) locations in Texas. The OT evaluates and treats patients with physical, developmental, cognitive, or functional limitations, develops individualized plans to improve daily activities, independence, and quality of life, and collaborates with physicians and care teams.

Qualifications

• Graduate of an accredited School of Occupational Therapy.
• Current licensure as an Occupational Therapist in the applicable state of practice.
• Ability to communicate effectively in English, both verbally and in writing.
• Strong problem-solving skills to assess patient needs and implement interventions.
• Strong communication and interpersonal skills with patients, families, staff, and teams.
• Ability to meet health, licensing, and employment requirements.
• Ability to perform essential job duties with or without accommodation.

Responsibilities

• Screen, evaluate, and treat patients requiring occupational therapy services.
• Complete comprehensive OT evaluations using appropriate tools and procedures.
• Review patient records and history to support treatment planning.
• Establish measurable goals and develop individualized treatment plans.
• Supervise OT Assistants and Rehabilitation Technicians per state regulations.
• Communicate findings and progress to patients, families, physicians, and team.
• Collaborate with physicians and healthcare teams to adjust plans of care.
• Maintain accurate documentation of evaluations, treatment records, and discharge summaries.
• Participate in care conferences, meetings, and care coordination activities.
• Educate caregivers and family members on maintenance programs and techniques.
• Serve as clinical preceptor for OT education programs.
