Job summary

Cantex Continuing Care Network is seeking a licensed Occupational Therapist to provide home health services. The OT will evaluate patients, develop plans of care, implement interventions, and monitor progress to restore independence and safety in daily activities.

Responsibilities include timely evaluations, thorough documentation, care plan updates, and ensuring regulatory compliance while educating patients and families. Home health experience and CPR certification are preferred.

Qualifications

• Strong communication, documentation, and assessment skills.
• Experience or knowledge in social work methods.
• Home health experience preferred.
• Knowledge of Medicare home health documentation preferred.
• Current CPR certification preferred.
• Ability to read, write, and speak English.

Responsibilities

• Screen, evaluate, and treat patients requiring occupational therapy services.
• Complete occupational therapy evaluations within three working days of Start of Care using appropriate evaluation tools.
• Review patient records and medical history to support treatment planning.
• Establish measurable, functional goals and develop plans of care.
• Coordinate care with interdisciplinary team and physician.
• Supervise OT services and COTA supervision as required.
• Document visits, medical necessity, and interventions accurately.
• Perform re-evaluations and modify plans of care as needed.
• Participate in interdisciplinary communication and discharge planning.
