Description
Treat patients as directed by the Occupational Therapist. Record daily treatment notes and weekly progress notes per OT Board. Treat patients per the physician treatment plan. Assist nursing department with training of Restorative Aides. Communicate with supervisor and other health team members regarding patient progress, problems, and plans. Participate in in-service training programs for staff in the facility. Record evaluations, treatment notes, recertification documentation, and progress notes, complying with Occupational Therapy Board State Practice Act and third-party payer requirements. Record treatment changes per policy and procedures. Instruct patients’ families or nursing staff in maintenance and caregiver training in preparation for discharge from therapy services. Participate in discharge planning. Assist with cleaning and maintenance of the treatment area. Report any equipment problems to ensure proper maintenance. Ability to relate positively and effectively with patients, families, staff, and colleagues.

Qualifications
Physical Demands
Frequent standing/walking, occasional sitting, frequent reaching, pushing, pulling, talking, and smelling. Ability to exert significant force and perform physical activities like climbing and stooping occasionally. Good vision and focus adjustment required.
Work Environment
