Job summary

LiveWell Homecare Agency seeks a licensed Occupational Therapist to assess, plan, and implement rehabilitation programs that restore daily activities, mobility, cognition, strength, and range of motion for patients in home and care settings.

You will collaborate with physicians, educate patients and families, document progress, and ensure infection control and confidentiality. This role emphasizes compassionate care, teamwork, and ongoing professional development.

Qualifications

• Graduate of an accredited occupational therapy program; Bachelor’s or Master’s degree required.
• Licensure as an Occupational Therapist and BLS certification required upon hire.
• Effective communication, customer service orientation, teamwork, and basic computer skills.

Responsibilities

• Assess patients in collaboration with physicians to develop and evaluate treatment plans, goals, and discharge strategies.
• Implement and adjust therapy interventions, selecting appropriate activities to meet patient goals.
• Educate patients and families, providing discharge instructions, follow-up, and community resource referrals.
• Document evaluations, treatment sessions, education, and discharge plans per departmental standards.
