Job summary

Brio Living Services in St. Joseph, MI is seeking an Occupational Therapist to join the PACE of Southwest Michigan team.

You will perform participant-centered evaluations, develop treatment plans, and provide OT services to help participants maintain independence, safety, and quality of life at home and in the community. The role requires an active Michigan OT license and at least 2 years of experience, with a valid driver’s license and CPR certification.

Qualifications

• Active Michigan Occupational Therapy license.
• Minimum two years of OT experience.
• Valid driver's license and reliable transportation.
• CPR certification or willingness to obtain.

Responsibilities

• Conducts initial assessments and reassessments of participant’s condition and functional status.
• Develops treatment programs and sets goals; provides occupational therapy.
• Reviews assessments and discharges participants when appropriate.
• Assesses need for adaptive and durable medical equipment.
• Communicates participant changes to IDT members; coordinates with Home Care Coordinator.
