Job summary

Andwell Health Partners is seeking an Occupational Therapist for a per diem role in Presque Isle, ME, serving the Presque Isle area and surrounding towns. The role focuses on community-based home health care, with a plan of care developed in collaboration with clients and families.

Qualified applicants hold a Master’s or Doctorate in Occupational Therapy, Maine OT license, and current CPR. A reliable vehicle and Maine driver’s license are required; home health experience is preferred.

Qualifications

• Master’s and/or Doctorate in Occupational Therapy from an accredited institution.
• One-year clinical Occupational Therapy experience preferred. Home Health experience also preferred.
• Current O.T. License in State of Maine & current CPR Card.
• Must have a reliable and insured automobile and a current Maine driver’s license.

Responsibilities

• Develop occupational therapy plan of care with measurable outcomes with client/family and physician, per protocols.
• Provide education and promote client’s rights and responsibilities in the plan of care.
• Develop, implement, and revise a plan of care aligned with client’s age, condition and needs.
• Implement care activities based on goals towards independent living for client/family.
• Continuously assess client needs and evaluate effectiveness of interventions.
• Refer to community resources to meet client outcomes when appropriate.
• Initiate diagnostic, preventive and rehabilitative procedures to aid care plan development.
• Evaluate and document progress and coordinate discharge with client, family, and health professionals.

What You’ll Do

• Develop occupational therapy plan of care with realistic/measurable outcomes in collaboration with the client/family and physician, including discharge goals and according to Organization protocols.
• Provide education and promote client’s rights and responsibilities for informed decision making in the plan of care.
• Develop, implement, and revise a plan of care that incorporates goals consistent with the client’s age, condition and needs.
• Implement client care plan activities based on treatment goals towards independent living for the client/family.
• Continually assess the client’s needs for occupational interventions and evaluate the effectiveness of the interventions on the occupational visit report.
• Refer to internal and external community resources to meet client outcomes when appropriate.
• Initiate diagnostic, preventive and rehabilitative procedures as appropriate to the client’s care & safety to assist in the development and revision of Plan of Care.
• Evaluate and document progress toward outcomes and coordinate discharge plan with client and family and other health professionals.
