Job summary

Andwell Health Partners in Lewiston, Maine seeks an Occupational Therapist to split time between the Outpatient Clinic and the Community. You will develop care plans, educate clients, and work with families and physicians to promote independence at home.

Requirements include a Master’s or Doctorate in OT, Maine license, CPR, and a valid Maine driver’s license. Home health experience preferred; you’ll thrive in a flexible, supportive team with opportunities for growth.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited institution.
• One-year clinical Occupational Therapy experience preferred. Home Health experience also preferred.
• Current O.T. License in State of Maine & current CPR Card.
• Must have a reliable and insured automobile and a current Maine driver’s license.

Responsibilities

• Develops occupational therapy plan of care with measurable outcomes in collaboration with client/family and physician.
• Provides education and promotes client rights and responsibilities in the plan of care.
• Develops, implements, and revises a plan of care aligned with client’s age and needs.
• Implements plan activities toward independent living for client/family.
• Assesses needs for occupational interventions and evaluates effectiveness on the visit report.
• Refers to community resources to meet client outcomes when appropriate.
• Initiates diagnostic, preventive and rehabilitative procedures as appropriate to care & safety.
• Evaluates and documents progress toward outcomes and coordinates discharge with client, family and health professionals.
