Job summary

Aspire Home Healthcare of Northern California Inc. seeks a licensed Occupational Therapist in Yuba City to provide per-visit therapy services. The role supports patients with a progressive treatment plan and coordinates care with the rehabilitation team.

Part-time or full-time options are available, with mileage reimbursement and merit-based evaluations. A driver’s license, CPR, and TB tests are required, and orientation at our Yuba City office is included.

Qualifications

• Licensed OT in CA is required.
• Minimum 1 year OT experience preferred.
• OASIS and WellSky experience strongly preferred.
• Physical within 6 months prior to first patient contact.
• Current TB results within last 12 months and CPR Certification.
• Valid driver’s license required.
• Ability to attend 1-day orientation at our Yuba City office.

Responsibilities

• Provide therapy services per Plan of Care and treatment goals.
• Plan and supervise COTAs and HHAs per treatment plan.
• Evaluate patient condition and document progress daily.
• Develop and adjust treatment plans and goals for each patient.
• Coordinate total patient care and maintain confidentiality.

This is a paid per visit position!

Aspire Home Healthcarewas provides patients with around the clock care and support. The OT interprets and evaluates recommendations and orders. Assigned patient’s capabilities and disabilities, plans and carries out a progressive treatment program, and coordinates treatment program with total rehabilitation program.

Duties Include:

• Effective patient care using approved treatment techniques and procedures. Evaluation of patient’s condition, functioning level and treatment goals and keeping current records.
• After evaluating the patient, the therapist is responsible for planning administering and supervising the COTAs and HHAs in accordance with the treatment plan.
• Administers appropriate evaluations to assess patient’s level of function and develops therapy goals. Develops treatment plan for each patient and performs appropriate individual treatment of assigned patients. Documents patient’s treatment in medical chart daily.
• Will provide therapy services as outlined in the Plan of Care.
• Assists patient and family with development and review of home program, evaluation and review of adaptive equipment needed in home setting and evaluation of onsite home setting when necessary.
• Charts services the Therapist has rendered to the patient, notes changes in the patient’ condition and/or family and home situation, makes revisions in the care plan as needed, records supervisory visits, evaluates patient care and progress and closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.
• Maintains confidentiality of client’s situation, records and services provided according to HIPPA regulations
• Evaluates the effectiveness of his/her therapy service to the individual and family.
• Submits all patient notes and/or visit records on a timely basis.
• Obtains orders for paramedical services and submits referral to the Director of Clinical Services, i.e., nursing services, medical social services, etc.
• Coordinates total patient care.
