Job summary

Aventura at Humility House in Austintown, OH is seeking a Full-Time Occupational Therapist to join our rehabilitation team. New grads are encouraged to apply and you'll work within a supportive interdisciplinary team in a Skilled Nursing and Long-Term Care setting.

You'll evaluate residents, plan individualized OT care, and address ADLs, mobility, and safety, with opportunities for professional growth. In-house documentation and collaboration are core aspects of the role.

Qualifications

• Graduate of an accredited Occupational Therapy program and eligible for Ohio licensure.
• New graduates welcome and encouraged to apply.
• Current Ohio Occupational Therapist license or eligibility required.
• Experience with SNF/LTC settings a plus, but not required.
• Strong communication and organizational skills; team-oriented.
• Familiarity with Net Health and PCC is a plus.

Responsibilities

• Evaluate residents and develop individualized OT plans of care.
• Provide skilled OT interventions addressing ADLs, mobility, strength, safety, and independence.
• Establish goals and monitor progress with residents.
• Document accurately in Net Health in a timely manner.
• Collaborate with nursing, therapy, physicians, and the team.
• Supervise and provide direction to COTAs as appropriate.
• Educate residents and families about treatment plans and recommendations.
• Participate in discharge planning and safe transitions.
• Maintain compliance with state, federal, facility, and professional standards.

What You'll Do

As an Occupational Therapist, you'll help residents maximize their independence, safety, and quality of life through individualized, resident-centered care.

Responsibilities include:

• Evaluate residents and develop individualized occupational therapy plans of care

• Provide skilled OT interventions addressing ADLs, functional mobility, strength, safety, and independence

• Establish appropriate goals and monitor resident progress

• Complete accurate and timely documentation using Net Health

• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team

• Provide supervision and clinical direction to COTAs as appropriate

• Educate residents, families, and caregivers regarding treatment plans and recommendations

• Participate in discharge planning and promote safe transitions

• Maintain compliance with applicable state, federal, facility, and professional standards

Qualifications

• Graduate of an accredited Occupational Therapy program

• Current Ohio Occupational Therapist license or eligibility for Ohio licensure

• New graduates are welcome to apply

• SNF/LTC experience is a plus, but not required

• Familiarity with Net Health and PointClickCare (PCC) is a plus

• Strong communication, clinical, and organizational skills
