Job summary

The Grand Rehabilitation and Nursing at River Valley seeks a compassionate Occupational Therapist to join our rehabilitation team in Poughkeepsie, NY. You will evaluate residents, develop and implement treatment plans, and monitor progress to help residents regain independence.

Qualified candidates have an accredited OT degree and NY state OT license; experience is a plus. We offer competitive compensation, a strong benefits package, and opportunities for professional growth and training.

Responsibilities

• Evaluate and develop treatment plans for residents based on medical diagnosis.
• Implement treatment plans to improve residents' functional independence.
• Assess and document progress of residents in detailed re-certifications, progress reports, and daily notes.
• Maintain documentation of Resident care services by auditing Resident & Department records.
• Supervise occupational therapy assistants and participate in care plan meetings and documentation.
• Plan, develop, and organize OT-related reports and data tracking as directed by the Rehab Director.
• Assist in departmental projects as needed.
