Victoria Care Center in Ventura, CA is a skilled nursing facility with long‑term care, post‑acute rehab, and geriatric outpatient therapy services.
Flexible PRN schedule. Pay: $47 – $55 per hour, based on experience.

Key Responsibilities
- Screen/evaluate patients with cognitive and/or ADL functioning disorders and develop appropriate plan of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence‑based practice to support clinical interventions.
- Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL functioning disorders per regulatory and clinical practice requirements.
- Administer procedures as part of the rehab plan, including ADL retraining, therapeutic activities, exercises, modalities, supportive and assistive devices, etc.
- Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness; modify services and make referrals as appropriate.
- Provide consultation and counseling to patients, families, caregivers and other service providers.
- Communicate with supervisor and health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summary of services per regulatory and clinical requirements.
