Flagship Therapy is looking for an Occupational Therapist - PRN to join our Victoria Care Center in Ventura, CA. In this role, you will assess and treat patients with cognitive and ADL functioning disorders while being part of a dynamic team.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL functioning disorders.
• Utilize standardized assessments to support clinical interventions.
• Provide rehabilitative treatment interventions per regulatory requirements.
• Administer procedures as part of the rehab plan.
• Assess patient progress and adjust treatments accordingly.
• Evaluate treatment outcomes for effectiveness.
• Provide consultation and counseling to patients and families.
• Communicate with supervisor and health team members.
• Provide comprehensive discharge summary of services.

Key Responsibilities

• Screen/evaluate patients with cognitive and/or ADL functioning disorders and develop appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL functioning disorders per regulatory and clinical practice requirements.
• Administer procedures as part of the rehab plan, including ADL retraining, therapeutic activities, exercises, modalities, supportive and assistive devices, etc.
• Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness; modify services and make referrals as appropriate.
• Provide consultation and counseling to patients, families, caregivers and other service providers.
• Communicate with supervisor and health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services per regulatory and clinical requirements.
