Job summary

Vintage Faire Nursing & Rehab in Modesto, CA seeks a full-time Occupational Therapist to join our in-house rehab team. We offer a $10,000 sign-on bonus and an hourly pay range of $55–$70 based on experience. Flexible scheduling and a comprehensive benefits package accompany this role in a geriatric rehab setting.

Join our expanding in-house therapy team and contribute to clinical mentorship, program development, and patient-centered care in a supportive environment.

Qualifications

• Licensed Occupational Therapist required.
• Open to applicants of all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders and develop a care plan.
• Use standardized assessments and evidence-based practice to guide clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment for cognitive/ADL disorders per regulatory and clinical requirements.
• Administer rehab procedures including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate outcomes and refer to services as needed.
• Provide counseling to patients and families and coordinate with the health team.
• Communicate progress with supervisor and team and deliver discharge summaries.

Our expanding network of in‑house therapy Ensign‑affiliate operated healthcare facilities is growing, and we are scouting for excellent therapists to join us at Vintage Faire Nursing and Rehab. We offer a stable and fun in‑house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking full‑time Occupational Therapists for our in‑house rehab program!

Key Responsibilities

• Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including but not limited to: ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provide consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
