Job summary

Best Home Health Providers Inc. is seeking a per diem Occupational Therapist to support patients in their homes across the Bay Area and beyond.

You will evaluate functional abilities, create individualized plans, and guide patients in daily living activities while coordinating with physicians and the care team. The role requires an active California OT license, at least 2 years of OT experience, and a current CPR certification.

Qualifications

• Minimum 2 years of clinical OT experience.
• Active California Occupational Therapy license.
• CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle.
• Willingness to travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living activities affected by illness or injury.
• Instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
