Job summary

Best Home Health Providers, Inc. is seeking an Occupational Therapist to support patients recovering at home across the Bay Area. You will evaluate functional abilities, create personalized plans, and guide patients to regain independence in daily living activities.

Responsibilities include coordinating with care teams, educating families, and supervising OT Assistants. The role offers per diem work with a $130 per visit rate, with potential to convert to full-time based on performance.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA Committee and AOTA.
• Registered with the NBCOT after graduation.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• CPR certification with AHA.
• Valid driver’s license and insured vehicle, travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate with physicians and care team.
• Teach and retrain patients in daily living skills.
• Recommend adaptive equipment and assistive devices.
• Maintain accurate clinical records and report changes promptly.
• Coordinate with the care team for seamless, patient-centered care.
• Educate family members and caregivers on treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service.
• Support discharge planning as patients transition home.

• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
