Best Home Health Providers is seeking an Occupational Therapist for a per diem role delivering in-home rehabilitation across the Bay Area. You will assess functional abilities, create personalized plans, and educate patients and families on daily living activities.

The role requires an active California OT license, at least two years of clinical OT experience, and CPR certification. Travel to patient homes is expected, with potential to transition to full-time based on performance.

Responsabilidades

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

Formação académica

OT curriculum accreditation (AMA/AOTA)

California OT license (active)

Registered with National Registration Examination (AOTA)

Minimum 2 years OT clinical experience

CPR certification (AHA)

Valid driver’s license with reliable insured vehicle

Please note that this role is per diem with a rate of $130 per visit.

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
