Job summary

Best Home Health Providers is seeking a per diem Occupational Therapist to support home-based patient care across the Bay Area. The role focuses on evaluating functional abilities, creating individualized treatment plans, and guiding patients to regain independence within their own homes.

Applicants should hold an OT degree, CA licensure, and NBCOT registration, with at least two years of clinical OT experience and CPR certification.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA and AOTA.
• Registered with the NBCOT/National Registration Examination for OT.
• Active California OT license required.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license with reliable insured vehicle and ability to travel to patient homes.
• Flexible schedule and strong communication/or organizational skills.
• Ability to convert to full-time based on performance.
• Paid sick days and paid bereavement leave.
• Mileage reimbursement and referral bonus available.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $110 - $130 per visit.

• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
