Job summary

Angels Care Home Health is seeking an Occupational Therapist (OT) to join our team in Troy, MO. The role focuses on clinical assessment, developing and updating the Plan of Care, and documenting progress notes to support patient goals and outcomes.

Responsibilities include applying infection control practices, coordinating care with the team, and adjusting the Plan of Care based on ongoing assessments. A current OT license, CPR, and DME knowledge are required to ensure high-quality patient care.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment and identify patient needs.
• Prepare, complete and update a Plan of Care and progress notes.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in patient care.
• Accept assignments within education and competence to care for patients.
• Assess physical, social, and cultural components of the patient’s occupational environment.
