Job summary

Angels Care Home Health is seeking an Occupational Therapist to perform assessments and develop, implement, and revise patient plans of care in a home health setting in Nebraska. The role requires collaboration with physicians and the health team to ensure high-quality, patient-centered care.

Qualifying candidates hold an active OT license, have at least two years of experience, maintain CPR and Hepatitis profiles, and are knowledgeable about durable medical equipment.

Responsibilities

• Perform clinical assessment and identify patient needs and appropriateness of care.
• Document, prepare, and complete the Plan of Care, progress notes, and other clinical records.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in patient care.
• Coordinate with the physician-directed plan of care and communicate with the care team.
• Assess the patient’s occupational environment including physical, social, and cultural components.
