Job summary

Dove Healthcare in Fennimore, WI is seeking an Occupational Therapist to join our team on a part-time basis. You will plan and direct OT services, participate in discharge planning, and maintain detailed patient records.

As An Occupational Therapist (OT), You Will

• plan, develop, organize, implement, evaluate, and direct occupational therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
• participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
• maintain treatment records, patient files, and progress notes as required
• participate in the development and implementation of patient assessments and care plans, including quarterly reviews

Required Qualifications

• master's degree (or bachelor's degree for those grandfathered in) in occupational therapy from an accredited college or university;
• current license as an occupational therapist in the State of Wisconsin
