Job summary

Northgate Health Care Facility in Tonawanda, NY is seeking an Occupational Therapist to join its rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide therapy to improve daily living skills and mobility.

This full-time position offers a $33/hour rate and a days schedule, with opportunities to collaborate with physicians, nurses, and COTAs. The facility is part of The McGuire Group, known for quality care.

Qualifications

• Master's Degree in Occupational Therapy from an accredited program (AOTA).
• Valid New York State Occupational Therapist License required.
• Newly licensed OTs are encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled therapy services focused on improving functional abilities, ADLs, mobility, and quality of life.
• Establish short- and long-term rehabilitation goals and monitor resident progress.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes.
• Supervise COTAs in accordance with state regulations.
• Complete all required documentation, assessments, care plans, and billing records accurately and timely.
• Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs.
• Coordinate adaptive equipment and DME needs.
• Participate in care conferences, discharge planning, staff education, and quality improvement initiatives.

RESPONSIBILITIES:

• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations
• Complete all required documentation, assessments, care plans, and billing records accurately and timely
• Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs
• Recommend and coordinate adaptive equipment and durable medical equipment (DME) needs
• Participate in care conferences, discharge planning, staff education, and quality improvement initiatives
• Perform additional duties as assigned

REQUIREMENTS:

• Master's Degree in Occupational Therapy from an accredited program approved by the American Occupational Therapy Association
• Valid New York State Occupational Therapist License required
• Newly licensed Occupational Therapists encouraged to apply
• Skilled nursing facility experience preferred
• Strong clinical, documentation, communication, and problem-solving skills
• Ability to work collaboratively within an interdisciplinary healthcare team
• Commitment to resident-centered care and rehabilitation excellence
